A cleanup rule isn't acting

In order: is it in safe mode (dry run)? Is the title excluded? Is someone mid-watch and holding it under multi-user protection? Is it inside a grace period? Did the run hit the blast-radius cap? The Dashboard's Next cleanup panel and the Leaving Soon queue show which of these applies.
